ABSTRACT
The purpose of this study is to assess differential diagnosis between primary, secondary and tertiary hyperparathyroidism, and to establish a protocol study and treatment y patients with primary lesion. We designed a retrospective-prospective review of 15 patients with primary hyperparathyroidism, with a mean age of 50 years. Every patient underwent surgical treatment, hystopathological study and measurement of calcium, albumin and phosphor serum concentration in the properative and postoperative time. Patients underwent image studies for diagnosis like ultrasound (US), computed tomography (CT), magnetic resonance imaging (MRI) and Sestamibi scan. Results: hystopathologicaly there were 13 parathyroid adenomas and 2 hyperplastic parathyroid glands. The serum calcium concentration preoperative were elevated in every case (9.6-13.5 mg/dl) and concentration of parathyroid hormone was the diagnosis study for primary hiperparathyroidism. The image studies performed in our patients were in 7 cases US, in 7 CT, none of this help us localize the affected parathyroid gland. MRI was performed in 3 patients and sestamibi scan in 10, this studies were determinant for the localization of the affected gland. We conclude that is necessary to make differential diagnosis between different types of hiperparathyoidism and the measurement of parathyroid hormone is the elective study for that. The image studies, once we have the diagnosis of primary hiperparathyroidism, are of vital importance because they help us to localize the affected gland, then we diminished the risk during surgery and surgery can be executed in less time.
