ABSTRACT
Coccidioidomycosis is an infection caused by the soil-inhabiting dimorphic fungus
Coccidioides immitis. This illness is endemic to certain regions from North America to Argentina. The endemic areas in the United States include southern states and the regions of Mexico that border with them. It’s been associated to the rain at the end of winter and spring, followed by droughts, with cases increased in autumn, associated with eddies and sand storms.
Case Report:An 1-year-old white female from de Sonora desert region, that comes to the hospital with productive cough and mild respiratory distress; she had two similar episodes in the past. She got worse during her hospitalization, x-ray images showed opacity of the middle lobe and miliar infiltrate. We found
C. immitis spherules in endobronchial brush sample besides coccidioidal immunoglobulin M (IgM) and complement fixation (CF). Antifungal therapy was initiated with amphotericin B desoxicolate in spite of wich she had no improvement, for that reason we added fluconazole.
Conclusion: In this case Coccidioidomycosis presented like a progressive pulmonary disease without response to initial amphotericin B desoxicolate treatment, in view of this we added fluconazole observing significant clinical, radiographic, and laboratory test improvement.
