Bone marrow biopsy: utility and limitations

ABSTRACT
We reviewed the uselfulness and limitations of the bone marrow biopsy in the study of patients with different primary and secondary benign and malignant processes. It is the most adequate and reliable procedure to study the bone marrow from patients with malignant tumors (mainly metastatic), marked cytopenias and leukoerythroblastosis in the peripheral blood, negative or insufficient bone marrow aspirates (“dry taps”), and in those with granulomatous diseases and fever of unknown origin. Its diagnostic uselfulness is limited in patients with hemolytic anemias, metabolic and lipid storage diseases, as well as myeloid and erythroid hyperplasias in peripheral blood. In the majority of the patients a well obtained and processed bone marrow biopsy stained with H-E and/or Giemsa and classic histochemistry (reticulin, iron, fungi and acid-fast bacilli), in conjunction with pertinent clinical information, allow the surgical pathologist/hematopathologist to make deffinitive diagnoses. Nevertheless, with a variable fre uency, it is necessary to use more specialized techniques (cytogenetics, flow cytometry, immunocyto/histochemistry and molecular biology) to stablish the diagnosis with precision.
