Invasive micropapillary carcinoma, an aggressive variant of breast carcinoma. Review of the entity in regard to 12 cases

ABSTRACT
Background: Invasive micropapillary carcinoma of the breast is a rare subtype of breast cancer with particular histologic features, related to more aggressive behavior and poorer prognosis.
Objective: To report the clinicopathologic and immunohistochemical features of 12 cases of infiltrating ductal breast carcinoma, with invasive micropapillary component.
Material and method: Twelve cases of breast cancer with invasive micropapillary carcinoma components from 2001 to 2006. Biopsies, mastectomized glands, and material from surgical pathology department of Centro Medico ABC, all studied through paraffin technique and hematoxiline-eosine stein.
Results: The mean age of patients was 50 years (range 26 to 88 years); most of cases were histological grade 2 or 3; every case showed lymphatic invasion; dissections of axillary lymph nodes in seven of eight cases showed metastasis; the number of positive lymph nodes ranged from 1 to 15; nine cases were positive for estrogen and progesterone receptors; seven cases were positive for CerbB-2, among these, 5 were highly positive (3+); p53 was positive in variable percentages; cellular proliferation index and vascular density were also variable; six of eight cases had abnormal nuclear ploidy.
Conclusion: A micropapillary component in a breast carcinoma is associated with a high risk of lymphovascular invasion and nodal metastasis, thus finding it is crucial for determining prognosis and an accurate patient management.
