Negative-pressure pulmonary edema and pulmonary hemorrhage, case report

ABSTRACT
Introduction: The acute pulmonary edema negative pressure is a complication described since 1977 due to the blockage of the airway breathing in children and adults. Although its etiology is multifactorial, highlights the excessive negative intrathoracic pressure caused by the forced spontaneous inspiration of a patient with a closed glottis, it results in transudate fluid from the capillaries into the pulmonary alveolar-interstitial space. The resulting pulmonary edema can occur within minutes after the airway obstruction or after several hours. The clinical manifestations are potentially serious, but usually responds well to treatment with oxygen, mechanical ventilation and diuretics. It is important to diagnose it to management immediately.
Objective: Report of a patient with acute lung edema by negative pressure in the immediate post with severe acute respiratory failure and pulmonary hemorrhage that required attention in the ICU.
Design: A case report.
Setting: ICU Hospital Regional de Alta Especialidad from Oaxaca.
Patient: male patient aged 42, farmer, scheduled for elective way to meningioma resection surgery, intubation a traumatic to the 1st attempt, during the surgery without complications, saturated to 98%, reporting bleeding of 400 mL. Awakening at the end of surgery it was extubated and go to the intensive care unit after surgery with fc: 80 x´ and SpO
2: 90%.
