Tomographic findings of micotic allergic sinusitis in patients with resistant to treatment chronic sinusitis

ABSTRACT
Background
The chronic sinusitis is the inflammation of the mucosa of the paranasal sinus of more than six weeks. When antibiotics are administered by four weeks without obtaining satisfactory reaction, treatment-resistant chronic sinusitis is considered. Mycotic allergic sinusitis is a relatively recent disease.
Objective
To determine the incidence and risk factors related to mycotic allergic sinusitis in patients with otorhynolaryngology service, Centro Medico Nacional Siglo XXI.
Material and methods
A retrospective, cross-sectional, observational and descriptive study was done in patients with treatment-resistant chronic sinusitis. Tomographic findings and magnetic resonance compatible with mtycotic allergic sinusitis, nasal poliposis, allergic rhinitis and asthma were analyzed. It was used descriptive statistics and percentage of the findings of mycotic allergic sinusitis was used.
Results
We registered 36 patients with treatment-resistant chronic sinusitis. All had antecedent of surgery; 22 (61,11%) had antecedent of atopy, 21 (58,33%) of allergic rhinitis, 5 (13,8%) of asthma and 3 (8.33%) of hypersensitivity to acetilsalicilic acid. The most frequent operation was the endoscopic one of the paranasal sinus, followed of the maxilar antrostomy, transnasal polipectomy and frontal osteoplasty. The main symptoms were: nasal obstruction (80,5%), hialine anterior rhinorrhea (36.1%), mucopurulent anterior rhinorrhea (27,7%) and mucopurulent posterior rhinorrhea (22,2%).
Conclusions
It is important to do a suitable interrogation and complete physical exploration, since they are indispensable to identify patients with treatment-resistant sinusitis. The opportune diagnosis will avoid subdiagnosis and will establish a better prognosis for cure.
