Proposal of nursery attention during the post-operative period secondary to Fontan´s surgery in pediatric patients

ABSTRACT
Patients subjected to Fontan’s surgery require specific care. At the mentioned surgery, pulmonary circulation is separated from systemic circulation provoking a series of physiologic and physiopathologic changes at the systemic and pulmonary levels.
Objective: To establish an infirmary attention plan from the identification of the most frequent clinical situations observed in patients who have undergone Fontan’s surgery.
Methodology: This was a descriptive, retrospective and longitudinal study. It was analyzed a population of 25 patients who had been operated through Fontan’s surgery during the period going from 2000 to 2006. The clinical records were reviewed in order to obtain quantitative parameters. They were analyzed by descriptive statistics methods. The clinical situations were deduced from the variables or parameters, establishing infirmary diagnosis and a proposition for a care plan.
Results: The most frequent clinical situations were: hemodynamic instability and a long use of mechanical ventilation, requiring sedation in the 99% of the cases; arterial hypotension in the 99% of the cases; continuous administration of inotropics in the 92% of the patients; and problems in gas interchange in the 72% of the cases.
Conclusion: Infirmary attention to patients subjected to Fontan’s surgery should be based upon a care planning and the priority of the most frequent problems. All these measures should allow to give effective and efficient attention.
