Strengthening of the Quadriceps in Gonarthrosis

ABSTRACT
Objective: Evaluation of a 6-month program of quadriceps muscle-strengthening with isometric and isotonic exercise combination in patients with osteoarthritis of the knee in a prospective, descriptive and auto controlled study.
Material and methods: Seventy eight female patients with mean age of 57 years (range, 49-75 years) with unilateral or bilateral osteoarthritis of the knee in radiologic grades I and II from Kelgreen classification.
Main measures: Gait speed, quadriceps isometric muscle strength, and pain qualification on an analog visual scale and daily analgesic requirement registry and functional knee evaluation with the International Knee Documentation Committee scale (IKDC).
Intervention: A combination of isometric and isotonic strengthening exercises was used for quadriceps muscle, with 5 sec by contraction in blocks of 10 movements against 50 and 90% of maximal resistance every 3 days during 6 months.
Results: Isometric quadriceps strength was increased in 200% (p = 0.05); the analgesic requirement was diminished from three doses/day to one dose/day in 56% of cases and 31% left them; increase gait speed was observed from 1.15 m/sec as mean to 1.29 m/sec due to larger step length. The program was considered according IKDC scale with excellent results in 30.5% of patients, good results in 38.5%, fair results in 23%, and poor results in 8% of cases.
Conclusions: The combination of isometric-isotonic quadriceps-strengthening exercises used in patients with osteoarthritis of the knee increases quadriceps strength, joint stability, and function-like gait, and helps to decrease pain and analgesic use.
