Postoperative complications in elders with elective non-cardiac surgery

ABSTRACT
Background: Perioperative management in elderly patients has suffered many changes. In our country, there are 6.9 millions of people of 60 years and older and by 2050 there will be 32 millions 400 thousands. Half of them will need a major surgery. If has been described that the most frequents postoperative complications in elderly patients are the cardiopulmonary. It is important to know about the kind and frequency of postoperative complications in our population.
Objective: To describe the postoperative complications in elderly patients submitted to elective non-cardiac major surgery.
Patients and methods: Men and women of 60 years and older who was submitted to elective surgery. We identified the comorbilities, kind of surgery, anesthesia, and postoperative complications. We employed descriptive statistics.
Results: 120 subjects, 70 men (58.3%) and 50 women (41.7%) with a mean age of 70 years old. Lower abdominal surgery was the most frequent (n = 35, 29.2%) and general anesthesia was the most employed (n = 84, 70%). 63 subjects (52.5%) had one complication and from whom, 52 (43.3%) had one comorbility as least. Complications found were metabolic (n = 75, 53.5%; hyperglycemia n = 36, 30%), cardiovascular (n = 23, 16.3%; systemic arterial hypertension (n = 19, 15.8%), renal (n = 20, 14.3%, acute renal failure, n = 20, 16.7%), infectious (n = 16, 11.4%), respiratory (n = 6, 4.2%) and we didn’t find neurologic complications.
Discussion: Metabolic postoperative complications were more frequent than cardiopulmony ones in elderly patients. Preoperative consultation, therefore, should be focus to identify patients with potential risk to present this kind of complications.
