Total Anomalous Pulmonary Venous Correction. A Case Report.

ABSTRACT
Introduction. Total Anomalous Pulmonary Venous Connection (TAPVC) is a complex heart disease. This rare condition has an
incidence between 0.4% and 2% in the first year of life. There is a marked male preponderance. This defect has no connection with
the left atrium. There’s a wide variety in their anatomic spectrum and clinical presentation depends on the grade of collector obstruction
or in the atrial septal defect size.
Clinical Case. A male newborn, obtained by C- Section, showed a respiratory distress syndrome and was diagnosed as a Transient
Thaquipnea (TT). Treatment consisted in IV fluid and oxygen. The respiratory rate was at 100 per minute, without insufficiency. The
outcome was not the expected, and a cardiology evaluation was solicited despite the abscense of murmur or cardiomegaly. An
echocardiogram was performed, observing an abscent connection of the pulmonary veins and the left atrium, the connection was
localized to the Portal vein, so we concluded a diagnosis of TAPVC to Porta Vein (TAPVC-PV). He went under surgical correction
the following day, with an excellent outcome and discharged after word .
Conclusions. TAPVC-PV as an infracardiac connection is a rare disease, and can easily be mistaken with respiratory disease as a TT
or Respiratory Distress Syndrome (RDS) and also with obstructive cardiological disease of the pulmonary veins. Echocardiogram is
still an important key to make a diagnosis and define the different anatomical types. Early diagnosis hence treatment of this important
for a succesful outcome.
