Treatment of incompetent perforator veins: subfascial endoscopical surgery vs ultrasound-guided sclerotherapy

ABSTRACT
Aim: To compare outcome of subfascial endoscopical perforator surgery (SEPS)
vs. ultrasound-guided sclerotherapy in treatment of incompetent perforator veins (UGS).
Methods: Design: Ambiespective, longitudinal, closed. Twelve patients with incompetent perforator veins with 13 limbs are included with clinical etiologic, anatomic and pathopysiologic classification (CEAP) 4 to 6, was evaluated before treatment, at 15 days and one month after treatment, whit venous duplex, CEAP, venous clinical severity score (VCSS) and venous disability score (VDS). Statistical analysis: Descriptive statistic and Fisher’s exact test.
Results: There were included eight limbs in group one treated with SEPS and five limbs in group 2 treated with UGS. Group 1 exist an occlusion of 100% of the incompetent veins and group 2 80% (p = 0.111). Heal of ulcer was 16.6% limbs at 15 days and 33.3% at one month in group 1 and group 2 there was not healing respectively. At evaluation with VCSS found group 1 outcome in 62.5% at 15 days and 87.5% at month, group 2 60 and 80% respectively (p = 0.685 and p = 0.641). Evaluation with VDS group 1 it has an outcome in 75 and 100% at 15 days and one month and group 2 60 and 80% (p = 0.510 and p = 0.111) at 15 days and one month, respectively.
Conclusion: The trends seems that there are better outcome in limbs treated with SEPS but there is not a statistic difference. It is necessary to increase the number of limbs included to obtain statistical difference.
