Insulin therapy in hospitalized patients with type II diabetes mellitus. Only option?

ABSTRACT
Objective: To determinate the prevalence of diabetes mellitus in hospitalized patients, demographics characteristic and treatment strategies.
Material and methods: A cross-sectional study, descriptive, in a cohorte of a convenience sample of patients admitted to an internal medicine department in a large tertiary care hospital. It included patients within 18 to 80 years, the presence or absence of diabetes, treatment, control and the existence of complications like hepatopathy and nephropathy was determined by laboratory exams.
Results: The total has 246 patients, 148 female (60%) and 98 male (40%), the average age was 54 ± 3; 42 patients had diabetes (17.07%), which 15 (35.71%) was under oral medication. The rest of the group used different regimen insulin protocols. The average ill length was 15 ± 4 years. In all individuals the fasting glucose control was superior at 110 mg/dl. 13 (30.9%) presented laboratory data compatible with complications like hepatopathy and nephropathy.
Conclusions: Diabetes mellitus prevalence was elevated in hospitalized patient, all of them with hyperglycemia and chronic complications; it made unjustified the use of oral hipoglucemiantes and a subutilitation of insulin. We propose standardized protocols with a multidisciplinary staff to improve the control of these patients.
