Clinical meaning and utility of Epworth Sleepiness Scale: new treatment proposal to patients with Obstructive Sleep Apnea Syndrome

ABSTRACT
Background: Polysomnography is the gold standard for diagnosis and treatment of Obstructive Sleep Apnea. Very few studies analyze the relationship between Epworth Sleepiness Scale and Apnea/Hypopnea Index.
Objective: To determine the statistical relationship between Epworth Sleepines Scale and Apnea/Hipopnea Index, including other clinical variables, to predict severity of the Obstructive Sleep Apnea syndrome.
Material and Methods: Retrospective cohort study in patients with history of nocturnal snoring, sleep apneas and excessive daytime sleepiness, including a complete PSG. A total of 52 patients were finally included in our study. Sex, age, ES total score, body mass index, other associated diseases and PSG parameters were registered. A statistical analysis using the Spearman correlation coefficient was done in order to find the relationship between the ESS and the variables previously mentioned.
Results: An important relationship was observed between the ES score and some parameters such as BMI. The only significant relationship, however, was seen between a score of 15 on the ES and an AHI of 15 or more (p ‹ 0.05, correlation coefficient was 0.769).
Conclusion: We suggest a management algorithm, “the rule of 15´s”, for patients in whom OSA syndrome is suspected, without the need to immediately order a PSG. This management algorithm is based on the present study´s statistical conclusions.
