Ambulatory surgery at a rural hospital: experience in vasectomy and bilateral tubal ligation

ABSTRACT
Background: The numbers of VAS have increased from 1976 to 1992 (0.6-1.4 %) and for TL (8.9-43%) during the same time period. We analyzed the experience of general surgeons (GS) on VAS and tubal ligation (TL) for male and female sterilization surgery (SS) in our rural hospitals (RH).
Methods: We analyze data regarding VAS and TL in two RH of San Luis Potosi, Mexico in a retrospective, observational and descriptive study of surgical patients operated on from 2001-2005 by a GS and by an obstetrician/gynecologist (Ob-Gyn). Variables analyzed were age, number of gestations, and adverse events; χ2 was used for statistical analysis.
Results: Of 19,622 procedures, we reviewed 4821 SS (24.6 %); 1942 procedure were performed by a GS (40.3 %) and 2879 (59.7 %) by an Ob-Gyn. There were 4687 TL and 134 VAS (all performed by GS with six complications). In the TL, there were 10 adverse events (GS, 0.55%) and 19 adverse events (Ob-Gyn, 0.65%), without statistical significance (
p = 0.5).
Conclusions: TL and VAS comprised 25% of surgeries performed in our RH. GS performed 40% of SS without significant differences from surgeries performed by the Ob-Gyn. SS performed by GS results in an optimal resolution for VAS and TL in our rural areas.
