ABSTRACT
The patient with chest trauma undergoing thoracic surgery could need the isolation of one lung in order to avoid bleeding to invade the contralateral lung or a separated and individualized distribution of the ventilation. It is absolute indication for tracheal or bronchial disruption and for high waste bronchopleural fistula.
The selective mechanical ventilation is a good procedure in the management of the patient with severe thoracic trauma, it’s characteristics must be individualized because of the phisiopathologic alterations caused by the trauma.
We will review the characteristics of the lung separation or one lung ventilation in thoracic trauma surgery adding our experience in the management of these patients in our hospital.
