Manejo del glaucoma neovascular refractario empleando la válvula de Ahmed. Experiencia clínica de corto plazo en un centro de referencia

ABSTRACT
Purpose: To assess the short-term efficacy and safety of Ahmed valve to control intraocular pressure (IOP) in eyes
affected by neovascular glaucoma.
Methods: A retrospective review of 521 consecutive cases of Mexican patients who underwent Ahmed valve implantation in one of their eyes due to congestive neovascular glaucoma was performed. Ninety five percent (n = 497) of the patients were followed-up at least three months.
Results: Pre-operative mean IOP (41.3 ± 12.7 mm Hg) was significantly reduced (P = 0.0001) at 3 months after surgery (17.7 ± 7.3 mm Hg). Overall surgical success one day, one week, one month, and three months after surgery was 80.4%, 86.8%, 80.2%, and 78.8%, respectively. Surgical success was more common among patients that underwent pan-retinal photocoagulation prior to valve implantation as compared to those that did not (P = 0.0027). Most common complications were encapsulated bleb (13.2%), anterior chamber narrowing (9.9%), hypotony (2.7%), and hyphema (1.5%).
Conclusions: The Ahmed valve implant is an effective and relatively safe device for controlling IOP in Mexican patients with congestive neovascular glaucoma, at least in the short-term.
