ABSTRACT
Objective: To demonstrate the etiology and pathogenesis of lateral nystagmus (LN) related to patients with a strabological syndrome with congenital esotropia and variability (SEVA) showing horizontal no-comitance movement (IHA), "billiard ball" phenomenon (FEBB) and latent nystagmus.Material and methods: This is a prospective, transversal, descriptive and observational investigation in SEVA. We explored the horizontal movement under infra-red light to register the different phases and characteristics of LN. Mode 1: Free movement. Mode 2: Movement under application of an occlusive object to the outside movement eye; Mode 3 plaque between both eyes. We made the correlation with electrooculography (EOG) and brain map.
Results: We studied 11 patients. The electroencephalography and brain map was correlated to a failure in time of stimulation between both hemispheres, and EOG registered negative waves in Q phase.
Conclusions: The cause of LN in SEVA was related to loss of synchronism between hemispheres registered in the beginning of the movement ("disynchronous"). LN is happening secondary to increase of wanted to see, modes 2, 3, and tendency to avoid the nystagmus when demand to see (mode 2). To propose a real treatment directed to the cause, increasing the stimulus to lateral rectus can be obtained by fadenoperation in both medial rectus.
