Incidence of injuries in the biliary tract in patients with laparoscopic cholecystectomy in the «Ignacio Zaragoza» Hospital

ABSTRACT
This objective in this study was to find the frequency of injuries in the biliary tract and the surgical treatment in patients in laparoscopic cholecystectomy in the «General Ignacio Zaragoza» Hospital of the ISSSTE in the period from November 1 of 1994 to March 31 of 2007. The study was retrospective, cross-sectional and observational of 3137 patients of both gender, between 15 and 98 years of age, identifying by intraoperative cholangiography, iatrogenic injury of the biliary tract. The injuries were classified with Strasberg and Bismuth scales. Twenty patients with biliary injury were identified, which represents a frequency of the 0,63%, 14 female patients and 6 male. The injuries were: 1 of the right hepatic conduit; 1 in an anatomical variant that ended at the union of the gallbladder with the cystic conduct; 12 patients with partial injury of the biliary tract, localized 7 in the hepatocholedoco and 5 in the supraduodenal choledoco; 6 with complete section of choledoco, classified in 2 of choledoco, 1 in level of the carina; 1 to the carina; 1 above of the carina and 1 complete section of the right hepatic conduct and injury of choledoco. The surgical treatment consisted in 3 hepaticojejunoanastomosis with Roux en Y, 3 intrahepaticojejunoanastomosis with Roux en Y, 11 primary repair with put in of T-tube and 3 primary suture. The main complication of the procedure is the iatrogenic injury of the biliary tract. The frequency of these injuries in the Zaragoza Hospital was 0,63%, which is within the international ranks.
