ABSTRACT
Thyroid nodules are a common clinical problem. The need to effectively distinguish between malignant and benign nodules becomes important to avoid expensive and unnecessary procedures and/or treatment of patients with benign thyroid disease as well as to assure appropriate and timely management of patients with thyroid cancer. All available data confirms that post-test likelihood for cancer, in the setting of a negative MIBI scan, is 0%. A negative MIBI scan excludes differentiated thyroid cancer and medullary cancer in all patients with a hypofunctioning thyroid nodule (negative predictive value is 100%). Patients with a hypofunctioning thyroid nodule and differentiated thyroid cancer or medullary cancer are › 98% of all primary thyroid cancer patients. Available data indicates that a patient with a HTN and a negative MIBI scan in whom there is no clinical or other reason to consider anaplastic thyroid cancer can have total peace of mind regarding the possibility of thyroid cancer.
