ABSTRACT
The development of video and endoscopic instrumentation systems have permitted diagnostic and therapeutic thoracoscopy for a great variety of procedures. Selective mechanical ventilation with double lumen tubes allow us to collapse the lung of the side where surgery is performed, making it easier for the surgeon to preserve the anatomic structures by making a better surgical dissection and obtaining all the benefits of minimally invasive thoracic surgery. Some of these benefits are: shorter length of stay, lower postoperatory pain, lees use of narcotics and a better postoperative outcome, diminishing considerably the surgical procedure costs. We will review the characteristics of selective mechanical ventilation (for one lung) in video assisted thoracoscopic surgery and the anaesthetic management.
