Ketorolac association in late hematomas after reconstruction with breast expanders

ABSTRACT
Hematomas can develop slowly and without symptoms, or rapidly with complex symptomatology. In breast surgery hematomas usually occur around the first 24 hours after surgery and their evacuation is mandatory, under optimal conditions. Some studies show the frequency of these hematomas is less than 2%. In this report we describe a case of slowly developing metachronic hematomas present after four and eight days following breast reconstruction with tissue expanders in a patient that underwent bilateral mastectomy. The patient did not report abnormal bleeding antecedents, hematic pathology, or taking of medications that would have impacted blood clotting or hemostasis. On the fourth day following surgery, the volume of rigth drainage was high with hematic aspect, forcing an exploratory surgery. Contrary to previously reported cases, on the eigth day the patient presented a similar drainage on the left side, that required one more exploratory intervention. Hematologic readings led us to discard bleeding of vascular origin, althought hematologic test were normal. The only thing we can associate as a risk factor is the use of tromethamine ketorolac in those days.
