Transluminal percutaneous ballon angioplasty in native aortic coarctation in adult patients: Experience to long term

ABSTRACT
Objectives: To show our experience in 60 patients with native aortic coarctation (CoAo) were subjected to transluminal Percutaneous balloon angioplasty (ATPB).
Material and methods: Of August of 1988 to Junio of 2004, 60 patient with native aortic coarctation were subjected to transluminal Percutaneous balloon angioplasty (ATPB). They were: 35 men (58.3%), 25 women (41.6%), with an average age of 24.6 ± 7.33 years (15 to 50). All of were arterial hypertension to the moment of the procedure, with a systolic pressure of 190 ± 32.3 mmHg. The gradient of systolic pressure (GPS) was of 74.49 ± 19.33 mmHg, a diameter of the ring of 4.25 mm ± 0.88 mm, the average of the used catheters balloon was of 17.5 ± 1.5 mm (15-20 mm). The diagnoses were made in the clinical evaluation, radiographic studies, echocardiography studies and tomography studies. Results: After the procedure, the GPS descended on the average to 7.76 ± 9.82 mmHg and the systolic pressure descended at 130 ± 22.86. The amplification of the ring from 4.25 mm to 15.61 mm.
Success: Dropped of 50% or more of the GPS, without complications. Success was obtained in 100%. You present a patient with complication minor: a hematoma that required surgical drainage. Follow-up: In the follow- up to 82 months ± 21.15 months. Thirty two patients have returned to repeated catheterization (65.5) being a GPS 7.05 + 13.66 mmHg, an increment of the diameter the ring is observed de 15.61 mm to 15.69 mm. Nobody patient has required new dilation. Aneurysms were not demonstrated in the site of the dilation. The rest of the intervened patients under control for the consultation with follow-up, echocardiography. Fifty three patients remain normal blood pressure without medical treatment and only 5 required alone one antihypertensive to low dose.
Conclusion: Our experience like in the literature suggests that the ATPB in CoAo is safe and very effective, with long term sustained improvement. No exist aneurysms. Most of the patients remain normal blood pressure to the follow-up.
