Pulsating pump of emptiness: alternative of extracorporeal support for extracorporeal membrane oxygenation (ECMO) and hemofiltration

ABSTRACT
The way to reproduce artificially vital functions for extracorpored support has been developed surprising. Extracorporeal membrane oxygenation (ECMO) and procedures of hemofiltration are carried out through a device system which allows the diffusion of blood through the systems developed for oxygenation and filtration., respectively. The development of an efficient device of extracorporeal pumping, would provide advantages for the procedures of ECMO, hemodialysis/hemofiltration, pumping in extracorporeal systems of membrane oxygenator and ventricular support. In the Hospital General Universitario “Gregorio Marañón” of Madrid, has been developed a pulsating pump of emptiness (BPV) controlled by a computer system. This device consists of an external rigid camera and an internal elastic tube which allows a comfortable distension within the camera. In the rigid camera a negative pressure is applied through a suction of an emptiness pump; it produces an expansion of the elastic tube and consequently a dynamic and comfortable manipulation of diastole and sistole. The use of this pump in protocols of experimental surgery has shown advantages in comparison with the pump of rollers (hypothermal perfusion of kidney) and it is a useful tool in renal replacement therapies and in the preservation of organs. In this issue we reviewed the basic characteristics of the BPV, its relevance in experimental surgery (small pigs) and its application in ECMO and hemofiltration.
