Adherence to recommendations on mechanical ventilation in the intensive care unit

ABSTRACT
Introduction: There are few data in Mexico about adherence to recommendations in mechanical ventilation (MV).
Methods: Prospective collection of data of all patients with MV in The ABC Hospital from March 1st to August 1st 2003, choosing mean ± SD of tidal volume per kilogram (ideal weight) (Vt/kg), PEEP, FiO2 and Peak Inspiratory Pressure (PIP) (first 72 hours). We divided patients in Group A: with ARDS and Group B: without it.
Results: Two hundred and three patients 11% with ARDS and 89% without. Vt/kg: days 1, 2 and 3: 8.2 ± 3.5, 7.8 ± 3.7 and 7.7 ± 4.3 mL (p ‹ 0.0001 between 1-2, p ‹ 0.018 between 2-3): PEEP: day 1, 2 and 3: 6.3 ± 4.2, 8.3 ± 5.3 and 8.2 ± 4.9 cmH2O, p ‹ 0.0001 between 1-2 and p ‹ 0.0001 between 2-3. A vs B: p significative the 3 days. FiO2: day 1: 63 ± 17, 2nd day: 52 ± 14 and 3rd: 53 ± 17% (p ‹ 0.0001 between 1-2 and p ‹ 0.001 between 2-3). A vs B: p = 0.001 for day 1; PIP: 22 ± 8 day 1, 26 ± 12 day 2 and 25 ± 10 cmH2O day 3. A vs B: p = 0.0001 and p = 0.004 days 1 and 3 respectively. Global mortality: 21%, group A 35%, B: 19%.
Conclusions: Exists adequate adherence to recommendations in MV for ARDS but there is not evidence of their utility in patients without ARDS.
