ABSTRACT
Pneumonia caused by
Pneumocystis jiroveci (PCP) has always been a major cause of illness and death in patients with HIV infection. The diagnosis of PCP is established by identification of the organism in specimens obtained from the respiratory tract, either in sputum or by bronchoscopy. Trimethoprim-sulfamethoxazole (TMP/SMZ) is the preferred treatment. Patients who do not respond or who are intolerant of this medication are usually given pentamidine. When the treatment of PCP is delayed or ineffective, patients may develop hypoxemic respiratory failure. The clinical and radiographic features of severe PCP resemble the acute respiratory distress syndrome, with hypoxemia, intrapulmonary shunting, reduced pulmonary compliance, and diffuse radiographic opacities. Pneumothorax is common, and is associated with poor prognosis. Corticosteroids may attenuate lung injury caused by the inflammatory response. The early use of highly active antiretroviral therapy (HAART) is the most important determinant of long-term survival. The case of a man patient, with HIV infection with PCP is presented. The findings in thorax radiography as in CT are discussed.
