ABSTRACT
Objective: To describe the present state of endoscopic cholangiopancreatography (ECP)
Data collection: Review of the relevant literature (47 references).
Selection of studies: We analyzed the articles related to the use of ECP in the most frequent problems encountered in the biliary tract and pancreatic duct.
Results: Since 1974, when the first endoscopic sphincterotomy was performed, the therapeutic possibilities of the procedure have widened allowing to resolve many of the obstructive problems encountered in the biliary and pancreatic tracts. Among the diseases of the biliary duct choledocholithiasis is one of the diseases in which the procedure is most often recommended with a greater than 90% success rate. The diverse biliary decompression methods allow to resolve totally or partially the obstructive problems, either benign or malignant, and in occasions it is possible to perform, in the latter, very effective palliation measures. ECP allows detecting and managing congenital malformations, some cases of acute pancreatitis, pseudocysts, tumors, and fistulas. Complications of the procedure are of around 3%, increasing to 8% when performing a sphincterotomy, with a mortality rate of 0.4%, mainly related to difficulties in cannulation and in the application of the pre-cutting technique. Contraindications are rare and mainly related with the general condition of the patient, little collaboration, or lack of consent.
Conclusion: Endoscopic cholangiography is one of the studies that allows us to identify with greater accuracy the anatomy of the biliary tract and the diverse pathologies affecting it.
