Intrinsic acute renal failure due to acute tubular necrosis (ATN) is associated with a varying degree of oliguria, rising serum creatinine levels, decrease of renal blood flow (RBF), large drop in glomerular filtration rate (GFR) and tubular disorders. Severe acute renal failure is a major component of the multiorgan failure syndrome and is associated with a mortality rate of 50% in critically ill patients, despite renal replacement therapies. Since renal hypoxia can play an important role in its pathogenesis, management goals include strategies to optimize RBF.3 Low-dose dopamine (LDD) selectively dilates renal vessels, increasing RBF, GFR and sodium excretion;REFERENCES
