Impacto de la extracción sanguínea en el paciente grave

RESUMEN
Objetivo. Evaluar el impacto de la toma de muestras sanguíneas en el paciente crítico y su relación con el descenso de la hemoglobina y requerimientos transfusionales.
Diseño. Estudio clínico prospectivo.
Sitio. Una UCI de un hospital de tercer nivel de México.
Pacientes. Cincuenta pacientes (33 pacientes quirúrgicos y 17 pacientes médicos), edad media 55 ± 19 años, escala de APACHE II 18 puntos. Todos los pacientes con sangrado activo o hemólisis durante el periodo de estudio fueron excluidos.
Intervenciones. Toma de muestras de sangre para determinación de hemoglobina, transfusiones sanguíneas.
Mediciones y resultados principales. La extracción de sangre en los 50 pacientes fue de 20.3 litros (promedio de extracción 20.3 litros/día), la hemoglobina disminuyó 2 g/dL y se administraron 50 unidades de paquete globular a 26 pacientes.
Conclusión. La extracción en el paciente crítico se asocia con anemia, que propicia el uso injustificado de transfusiones.
