Renal function in the critically ill patient with renal failure according the level of creatinine clearance

ABSTRACT
Objective: To analyze the clinical features of acute renal (ARF) of ICU patient according the levels of creatinine clearance (Ccr).
Design: Retrospective and transversal study.
Setting: ICU of a tertiary care hospital of Mexico City.
Patients: Seventy eight ARF patients divided in two groups according Ccr: Ccr ‹ 10 mL/min and Ccr 11-20 mL/min.
Measurements and main results: Although the causes of ARF varied, sepsis occurred more frequently in both groups. As compared to patients with Ccr ‹ 10 mL/min with patients with Ccr 11-20 mL/min, had significantly higher serum creatinine (p ‹ 0.001) and lower urinary volumen (p ‹ 0.001), urinary osmolarity (p = 0.002), free water clearance (p = 0.046) and fractional excretion of water (p < 0.001), had more organic failures (p = 0.04), required dyalisis more frequently (p = 0.001), and had a higher mortality rate (42.4%); p = 0.001.
Conclusion: ICU patients with Ccr › 11 mL/min have better tubular function as those patients with Ccr ‹ 10 mL/min and less morbidity and mortality.
