Estimation and measurement of energy expenditure in critically ill patients

ABSTRACT
Energy expenditure (EE) in critically ill patients is reviewed. Differences between predicted and measured EE in critically ill patients are emphasized. Predicted EE is based on standard mathematical formulas which take into account sex, age, weight and height derived from normal ambulatory patients. The values obtained from this formulas are considered the basal metabolic rate of normal individuals, and when applied to different pathological states, specially corrected factors are used, which makes them difficult to apply in critically ill patients since conditions vary widely from day to day. Measured EE by indirect calorimetry is considered the "gold standard", but it is expensive and not readily available in all intensive care units, so an alternative and practical method to predict energy expenditure using capnometry and the modified Weir equation developed in our intensive care unit is presented.REFERENCES
