Azul de metileno en choque séptico refractario

RESUMEN
Objetivo. Evaluar la utilidad del azul de metileno en un paciente con choque séptico.
Diseño. Reporte de un caso y revisión de la literatura.
Sitio. Una UCI de un hospital de tercer nivel de México, D.F.
Paciente. Una mujer de 75 años de edad que ingresó a la UCI por choque refractario.
Intervenciones. Ventilación mecánica, administración de líquidos, dobutamina, norepinefrina, azul de metileno, antibióticos, monitoreo invasivo y no invasivo.
Mediciones y principales resultados. Se administró azul de metileno, 2 mg/kg por vía intravenosa durante 180 minutos y se midieron parámetros hemodinámicos basales a los 15, 30, 45, 60, 120 y 180 minutos. Se observó un incremento de la presión arterial media, índice cardiaco, resistencias vasculares, disponibilidad de oxígeno y función diastólica. Sin embargo, más tarde se observó un deterioro progresivo cardiovascular y la paciente falleció.
Conclusión. El azul de metileno mejora transitoriamente el estado hemodinámico de los pacientes con choque séptico.
