ABSTRACT
Problems in transfusional medicine are common. We require evidencebased diagnostic and therapeutic progress information that may allow to find adequate solutions. Controlled random studies are scasce in our field, since transfusion needs are often critical. In 1818 Blundell reported satisfactory transfusion results in post-partum hemorrhage in several women. During the world wars, soldiers with severe wounds were saved with blood transfusion, as evidence of its efficacy. After 1945 hemotherapy was performed on children and adults. Other milestones in blood transfusion include the discovery of Dausset’s 1954 HLA system, the 1960’s blood component separation in plastic bags and Blunberg´s 1968, discovery of the Australian hepatitis B antigen. In 1947, 10g Hb/dL was proposed as desirable threshold for surgery. This idea still persists 40 years later. In 1999, Hébert ad colleges found evidence of successful transfusions with 7g Hb/dL in critical patients. The blood transfusion risks of the patient´s viral contaminations are continuously decreasing due to more sensitive and specific tests. Inactivation of blood pathogen microorganisms is another resource for transfusion security. Nevertheless, risks of blood identity confusion due to incompatible transfusions still persists. Post-operatory immunomodulation has been used in orthopedic surgery. Progress in donor selection methodology and conditioning of blood components, will favor the prevention of receptor transfusion risks.
