Acute renal failure in trauma

ABSTRACT
Rhabdomyolysis is the first cause of acute renal failure in the politraumatized patient. There are several mechanisms involved in the etiopathology of ARF in trauma: hypovolemia due to compartimental syndrome, myoglobin nephrotoxicity and derivates from heme group lipoperoxidation products. Clinical data are similar to other forms of acute renal failure, however there are some important differences like the presence of high serum levels of myoglobin and muscular enzymes. The basis of the treatment is the instauration of a volume restitution therapy and urine alkalinization, and in those patients in whom the medical treatment is overwhelmed the recommendation is the hemodyalisis therapy.
