Disseminated histoplasmosis in a patient with systemic lupus erythematosus

ABSTRACT
Opportunistic infection is common in patients with systemic lupus erythematosus (SLE). In some patients, it is difficult to distinguished between the effect of infection and exacerbation of SLE because both can produce similar symptoms. We present a case of a woman with fever who developed hemolytic anemia, lymphopenia, pleural and pericardial effusion, serum antinuclear antibodies and anti-DNA antibodies. She was diagnosed with SLE and received corticosteroids. After this, she developed disseminated histoplasmosis. Clinicians should be alert to the possibility of opportunistic infection in patients with SLE in cases characterized by fever, atypical clinical patterns, or poor reponse to immunosuppresive therapy.
