Cavitary pulmonary sporotrichosis: diagnosis and treatment

ABSTRACT
A 42-year-old male with primary pulmonary sporotrichosis, the diagnosis was confirmed by culture of
Sporothrix schenckii in the bronchoalveolar lavage sample, the patient had no dermatologic manifestation. The small number of cases with pulmonary involvement has prevented prospective clinical trials, to determine optimal therapy. The physician’s ability to prescribe the best treatment is limited by the scarce published experience with the use of the azole- drugs and low efficacy reported with older therapies. Oral itraconazol is recommended for patiens with non-life-threating pulmonary sporotricosis, while amphotericin-B is indicated for extensive pulmonary lesions. Surgical removal of infected lung tissue along itraconazol therapy appears to be the most effective treatment.
