Sutureless abdominal hysterectomy

ABSTRACT
Objective: To demonstrate the efficacy of the LigaSure Vessel-Sealing system as coadjuvant in total abdominal hysterectomy.Setting: General Hospital of Mexico.
Design: Prospective, longitudinal, randomized, comparative study.
Statistical analysis: Central tendency measures and Chi square.
Material and methods: Two-hundred-sixty-seven patients were subjected to programmed abdominal hysterectomies for diverse indications; they were divided in two groups: conventional hysterectomy with sutures (CH) and hysterectormy without suture, using Ligasure (LH). The studied variables were surgical time, transoperative bleeding and need of transfusing blood derivatives, and number of sutures used.
Results: Hundred-thirty-three patients were assigned to group CH and 134 to the LH group; mean age was of 55 years (39 to 71 years). Surgical indication was uterine myomatosis in 244 (91.4%) patients, surgical time was of 145 min for the CH group and 130 min for the LH group (p ‹ 0.05); transoperative bleeding was of 475 ml in CH and 330 ml in LH (p ‹ 0.05). Five cases of the CH group required blood transfusion, whereas in the LH group none needed it.
Conclusions: No significant differences were found regarding surgical time and amount of bleeding; in LH no transfusion of blood derivatives was required. Probably these variables are related to the size of the uterus, previous surgeries, and the ability of the surgeon. No complications were encountered and no re-interventions were needed.
