Postadenotonsillectomy hemorrhage in children

ABSTRACT
The files of 438 patients subjected to adenotonsillectomy between the years 2001 and 2005 were reviewed. Post-tonsillectomy bleeding requires either direct pressure or cauterization. Eight patients with a mean age of 4.3 years suffered profuse post-tonsillectomy hemorrhage between the fifth and the ninth day. Only one patient had Von Willebrand’s disease. The inferior pole of the tonsillar bed was the one to present bleeding in 50% of the cases. Bleeding was always controlled. A careful history to rule out a blood dyscrasia permits to expect an adequate hemostasis. A correct hemostasis is best assured in the operating room after completion of surgery while the patient is still under anesthesia. If the patient bleeds hours or days after surgery he must be studied with special laboratory tests in order to rule out any blood disease.
