Risk factors and management of infections in total knee arthroplasty

ABSTRACT
Objective. This paper has been intended to determine the risk factors at surgery for total knee replacement, as well as the frequency of infection and the effectiveness of treatment for complications.
Material and methods. It is a retrospective trial out of 447 patients who were operated on for total knee replacement (TKR) in a 10-year span. Comparison was designed between a group of 19 infected cases (4.2%) and a control group of 237 randomly selected non-infected patients for a whole 256. Age was 68 years in average.
Results. Average age in infected women was 71 years (non infected 60 years, p = 0.002). Average age for infected women and RA was 78 years (RA non infected 54 years, p = 0.0001). No difference in age existed in women who had arthrosis. Coexisting diseases determined no difference between RA and arthrosis in women. In men, difference found in those with infection was 59.6 years for RA and 72.6 for arthrosis (p = 0.0001). Infection was resolved in all patients as follows: Patelar component exchange in 1, total exchange in 2, debridement and flapping in 5 and arthrodesis in 10.
Conclusion. Main major risk factor for infection in TKR is age in women, when they are in their 8
th decade of life regardless of the existence or not of RA. Other major risk is RA in younger men. Such factors as arthrosis, coexisting diseases or simple aging in men do not appear to be a significant risk by themselves.
