Chondrolysis and acetabular erosion in the Lazcano’s hip hemiprosthesis with a following-up period from two to eight years

ABSTRACT
Objective. To
assess the outcome of the Lazcano partial prosthesis of the hip for acetabular protrusion at 2 to 8 years of follow-up.
Material and methods. This is a cemented monopolar partial chromo-cobalt prosthesis, which was designed with a femoral stem similar to that of Charnley, being provided by an anterior and posterior cobra-flange and a medial collar. Head is otherwise similar to that of the Moore or Thompson prosthesis. However, in order to avoid protrution into the acetabulum it is designed at 145-degrees of valgus instead of the standard 135. Out of 52 hips in 51 patients operated on from November 1991 through November 1997, 7 patients died in the following 24 months after surgery, remaining 45 hips for the present study. Average age was 74 years and follow-up 5 years and 3 months. Fractures were 1 basicervical, 9 Garden II, 5 Garden III and 30 Garden IV.
Results. Absence of acetabular wastening was observed in 36 hips (80%) while in 9 (20%) waste was from 2 to 5 mm. No case of acetabular protrusion was observed in the present series.
Conclusion. As far as present results can be addressed, this prosthesis must be advised as a resource of treatment for subcapital fractures of the hip with a reasonable expectancy of avoiding protrutio acetabuli, since the 145-degree increased valgus does not allow prosthesis to penetrate into the pelvis.
