Osteitis in adult people. A retrospective review on forty cases in lower extremities

ABSTRACT
Objective. The main objective of the present paper is to stress out on the importance of the prompt and early treatment of compound fractures since osteitis is the main chronic complication of either open or post-operative infected fractures, when oportune treatment is not given.
Material and methods. This is a retrospective trial of 40 patients, men and women, aged in average 47 years (19 to 81), who were admitted from July 1984 through September 1990 because of already established osteitis in lower limbs. Main treatment consisted on scarification in order to allow vascular tissue after removing nechrotic tissue. Stabilization was by external fixator and bony loss was treated eather by
“in situ” spongial bone chips or bone transportation according to the size of bone absence. When necessary, triple antibiotic scheme was applied.
Results. Osteitis became resolved in 36 out of the 40 cases (90%), however ample bony loss subsisted in two out of the 36, amputation was necessary in other two cases, one patient died and osteitis subsisted in one.
Conclusion. This is currently the best procedure for obtaining the bone healing when the exists bony loss and infection. Our results are similar to those reported by other autors.
