Metabolic evaluation of thyroid cyst

ABSTRACT
Introduction: The diagnostic point in the thyroid cyst knows the differential diagnosis between benign and malignant causes. The goal of this study was to assess the accuracy and complementary value to the fine needle aspiration biopsy (FNAB) of the metabolic evaluation in the thyroid cyst. Methods: Prospective, longitudinal, analytic, blind study in 140 non functional thyroid nodules, including only which was cyst in FNAB. We calculated the diagnostic value of the metabolic evaluation by Tc-99-MIBI scan. Statistical analysis was made through central tendency measures, sensitivity, specificity, accuracy, negative and positive predictive value (PV) and likelihood ratio (LR). Results: There were 26 (18.6%) cyst lesions. Twenty-four women and 2 male. Mean age 43.1 year (mode 55, range 17-63 years). The FNAB was diagnostic in 5 (19.23%); the MIBI scan was negative in 17 (65.38%) all benign and MIBI positive in 9 cases, 6 (23.08%) malignant and 3 (11.54%) benign. The MIBI diagnostic value was sensibility 100%, specificity 85%, PV+ 66.6% PV- 100% Accuracy 88.4%, LR+ 6.6 and LR- 0. Conclusions: Every thyroid cyst with FNAB non diagnostic should be evaluated by MIBI scan. All cyst lesions with a negative MIBI scan are 100% benign, all cyst thyroid lesion with positive MIBI scan should be further investigated due to the high probability of cancer.
