Incidence and mortality of fat embolism syndrome in Units of Intensive Cares of the Secretary of Health of the Federal District

ABSTRACT
Introduction: A minority of patients presents the fat embolism syndrome (FES) result of organic failure: pulmonary, cerebral and cutaneous, associated to fractures (Fx) bony; we do not know its real incidence.Objective: To know the incidence and mortality the FES in Units of Intensive Cares (UCI) of hospitals of the Secretariat of Health of the Federal District (SSDF).
Material and methods. Retrospective study in the General Hospitals Balbuena (HGB), Xoco (HGX) and La Villa (HGV) of the SSDF, reviewed of files of the patients entered to UCI during the 2005, diagnosed FES (criteria of Gurd-Wilson and Schonfeld). Statistic: descriptive and rate of incidence.
Results: The total of income, rate of incidence were 875 patients: 0.012 (12 patients with FES by each 1000 income); in HGX, HGB and HGV, rate of incidence: 0.004, 0.003 and 0.004 (4, 3, and 4 patients with FES by each 1,000 income) respectively. Age average: 38 years, 72% presented/displayed Fx of femur, score average (APACHE II): 23 points. Mortality occurred in: HGX and HGV: 2 patients by each 1,000 entered patients.
Conclusions: The diagnosis of the FES is mainly clinical, although probably we underestimated its incidence, to recognize it opportunely can diminish its mortality.
