Surgical treatment of the trochanteric fractures of the hip in the elderly

ABSTRACT
Hip fractures increase the mortality index in the elderly. Diagnosis is
made based on clinical history, symptoms, and simple X-ray frames of the
hip in two positions. We evaluated two surgical techniques for treating
trochanter unstable fractures in the elderly.
Methods: Patients
with unstable trochanter fractures with previous normal gait were
included. Fractures were classified by the Evans method modified by Jensen
and Michelson for unstable hip fractures. Patients were treated either by
using a partial hip prosthesis (Thompson or Lazcano’s), or by internal
fixation using a DHS system.
Results: 98 patients with a mean age
of 68 were treated. Patients in witch a partial hip prosthesis was used
could start rehabilitation almost immediately, or begin weight lifting
with the affected pelvic extremity. Patients with internal fixation using
DHS system could not lift weight until they had clinical and radiological
evidence of consolidation. The six month results did not differ.
Discussion: arthroplasty offered an earlier rehabilitation, and
could be considered an adequate option for treating trochanter hip
fractures in the elderly.
