Treatment of unstable vertebral cervical injuries

ABSTRACT
This is a series of 21 adult patients who had cervical spine injuries and were treated by anterior pure titanium plating and mono-cortical screws as well as bone grafting. Complete spinal cord injury was diagnosed at admission in 23.5% and it decreased after 15 months of follow-up to 17.6%. Neural integrity was present at admission in 29.4% an it increased at follow-up to 47.1%. Not absolute relationship was found according to the span between injury and surgery, since complete recovery was obtained as late as 60 days after injury and no recovery occurred even when surgery was performed in the first 24 hours; however no delay in surgery is still advised. Final outcome was better when steroids were employed in the spinal shock stage in patients with initial neural deficit. Sound fusion was obtained in all cases after anterior screw-plating in injuries of the cervical spine.
