Alternatives for abdominal wall repair in breast reconstruction with TRAM

ABSTRACT
TRAM flap breast reconstruction is an excellent option for women with breast cancer; however, it has a common complication: contour abnormalities of abdomen and abdominal-wall weakness. We studied 90 female patients undergoing TRAM flap breast reconstruction and their abdominal-wall complications. Their age ranged from 26 to 60 (average age 43 years). In all of them, the upper midline close was done with two layer stitch. The lower midline close was done in 5 patients (5.5%) without mesh and it results in 1 bulge (1.1%); in 36 patients (40%), we used only one mesh, and they developed 2 bulges (2.2%); no complications were encountered in last 49 patients (54.4%) with double mesh. We concluded that our double mesh technique, reinforce the abdominal-wall’s weaker site in patients with TRAM flap and this assure a better function and body contour.
