Fulkerson osteotomy in patients with patellofemoral pain and malalignment

ABSTRACT
Introduction. Fulkerson described the medialization and anteriorization of the tibial tubercle to treat anterior knee pain in patients with patellofemoral malalignment.
Objective. To assess the functional outcome of patients undergoing Fulkerson’s osteotomy.
Material and methods. Twenty patients treated with this method from January 1999 to January 2004 were studied. All of them underwent arthroscopy of the affected knee and lateral retinacular release. The clinical evaluation was based on a subjective pain evaluation, the Lysholm scale, the SF-36 and the KOOS questionnaire. Merchant’s patellofemoral congruence angle was used to make the radiological evaluation.
Results. The clinical assessment showed a statistically significant improvement in the four scales used, with an important increase from the preoperative to the postoperative score. The change in the patellofemoral congruence angle went from 36.5 ° 16.1 degrees to 33.2 ° 6.7, with a non-statistically significant difference.
Conclusions. The patients who underwent antermomedial transfer of the tibial tubercle had satisfactory clinical outcomes. The correction of the patellofemoral congruence angle improved, although not statistically significantly, suggesting that the medium-term improvement does not necessarily rely on an evident radiographic change.
