Description of advance care planning in older adult patients admitted to palliative care

ABSTRACT
Introduction: advanced care planning is important for planning future medical care when a person is unable to make decisions for themselves. In the case of older adults with limiting illnesses, it is crucial to have these advanced care planning established. The lack of information about who completes these documents within a hospital setting is an underexplored area. The objective of the study is to describe the clinical and functional characteristics of older adults who establish their advance care planning.
Material and methods: retrospective observational study of 139 participants over 60 years-old who carried out their advance care planning at Gea González Hospital between June 2021 and June 2023.
Results: it was found that most of the studied subjects had comorbidities, with sepsis being the main cause of death. The vast majority presented high palliative needs and functional dependency, so almost all decisions were made by their relatives, opting for less invasive methods. Finally, the average survival after the palliative care consultation was three days.
Conclusions: these findings underscore the importance of advance care planning prior to a hospitalization episode.
