PCR profiling of viruses and bacteria in respiratory secretions as a diagnostic method for pneumonia in pediatrics

ABSTRACT
Introduction: severe acute pneumonia is an important cause of morbidity and mortality, the use of antibiotics and resistance in the population is an issue that is becoming more and more important, which is why it is necessary to search for new diagnostic methods to streamline management.
Material and methods: a retrospective cross-sectional study was carried out applying the Bacterial Pneumonia Score scale to patients hospitalized in the years 2022-2023 at the Hospital Español, Mexico City, from their arrival to the emergency room, comparing the score with the results obtained from viral and bacterial PCR panels of respiratory samples.
Results: a total of 104 hospitalized patients with clinical criteria for pneumonia were included. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the respiratory viral and bacterial PCR panels were obtained. The viral panel showed sensitivity of 85%, specificity of 100%, PPV 41% and NPV 0%; the bacterial panel: sensitivity 33%, specificity 76%, PPV 32% and NPV 22%. A comparison of the scale with the positive results for viral and bacterial pneumonia was performed, reporting a significance of p = 0.003 for viral pneumonia and p = 0.000 for bacterial pneumonia.
Conclusion: the respiratory viral profile is more useful than the bacterial profile as a diagnostic guide for community-acquired pneumonia, always taking into account the clinical characteristics to guide treatment and describe epidemiological characteristics of the community. The bacterial profile requires clinical judgment due to the high percentage of nasopharyngeal colonization by pneumonia-causing bacteria.
