Electrocardiographic pattern De Winter, another severe and atypical form of acute coronary syndrome with ST elevation

ABSTRACT
Chest pain continues to be one of the main reasons for consultation in the emergency department, with a heterogeneity in clinical presentation that ranges from cardiogenic shock or cardiorespiratory arrest to benign non-cardiac pathologies; therefore, the electrocardiogram is an invaluable tool for diagnosis and risk stratification. Although ST elevation myocardial infarction is known to require urgent reperfusion, there are other high-risk electrocardiographic findings associated with adverse outcomes or suggesting the imminence of acute myocardial infarction, such as the De Winter electrocardiographic pattern. Therefore, a thorough initial clinical, electrocardiographic and enzymatic evaluation are beneficial elements to make an appropriate early diagnosis and avoid fatal complications.
