Nociplasticity, a contemporary perspective on pain

ABSTRACT
Nociplasticity represents a paradigmatic advancement in the understanding and management of pain, particularly chronic pain. This concept refers to the alteration of nociception that leads to an increased sensitivity to pain without clear evidence of specific tissue damage or known somatosensory nervous system dysfunction. It is distinguished from traditionally recognized nociceptive and neuropathic mechanisms, explaining chronic painful conditions that defy previous classifications. Its research focuses on the neuroplastic changes within the central nervous system (CNS) that result in an altered perception of pain, and the adoption of the term reflects an evolution in the understanding of pain, emphasizing the importance of neurobiological factors in the experience of chronic pain. As research progresses, findings must translate into improved clinical practices, offering hope to millions of people suffering from chronic pain without an identifiable physical cause. Nociplasticity not only represents a promising area of research but also serves as a reminder of the need for innovative and personalized therapeutic approaches in pain management. The purpose of this mini-review is to analyze, based on the literature, how the definition and inclusion of nociplastic pain into the pain classification were developed.
